Step by step

Check MEDPROS
Log in to MEDPROS with your CAC and see which items are red or due.

Ask about other red items
One red item, like hearing, vision, shots or dental, can block a PHA update at some clinics. Ask RHRP if anything else needs to be green first.

Do PHA Part 1 online
In MEDPROS Self-Service, complete PHA Part 1.

Book Part 2 and dental with RHRP
RHRP is run by Leidos QTC. Book on smp.qtcm.com or call 1-833-782-7477. You can pick an in-clinic visit, a virtual visit, or a unit group event.

Using your own dentist?
Print the current DD 2813 (NOV 2021). Have your dentist fill it out and sign it.
Older versions of DD 2813 are obsolete and may be rejected.

Send it in
Upload the DD 2813 on smp.qtcm.com or send it through RHRP. Keep a copy.

Check MEDPROS again
Check again in 2 to 4 weeks. If it is still red, give your copy to your readiness NCO.
Why it gets kicked back
- ! LHI is the old name. RHRP has been run by Leidos QTC since March 2023.
- ! Old DD 2813 forms are obsolete.
- ! Someone else has to enter civilian results into MEDPROS. Keep your own copy.
- ! One red item can hold up the rest.
